HHSG is not to be an employer with respect to your use of the Site and shall not be responsible for any employment decisions, for whatever reason made, made by any entity posting jobs on the Site. When you complete various forms provided on the Site, you may be asked to provide certain information including, without limitation, a valid name, address, phone number and email address (your "Information"). HHSG will not disclose to any third party your Information without your prior consent, except to the extent necessary or appropriate to comply with applicable laws or in legal proceedings where such information is relevant. HHSG may also provide your information to third parties as HHSG deems appropriate to complete transactions between you and our network of providers including advertisers and suppliers. HHSG reserves the right to offer third party services and products to you based on the preferences that you identify in your data submission and at any time thereafter; such offers may be made by the HHSG or by third parties. The Site provides a service where it matches requests from potential customers (you) with providers within our network, which may include those participating in third party networks, who supply medical, health care and other related products and services (the "Referral Service"). You hereby authorize HHSG to submit and share the information provided with contracted providers and referral network participants in accordance with these Terms of Use. You recognize that you have been informed through these Terms of Use of all disclosures required by law regarding the business relationship between HHSG, their participating providers and referral network participants. You further acknowledge that this authorization will remain effective unless you notify HHSG by email, phone or fax of the revocation of this Authorization Statement using the contact information provided on our Contact page. You further acknowledge that the information you provided is accurate and complete. The information you provide to us in a matching request will be sent to matching providers within the Referral Service network. HHSG is not involved in the actual transaction between customers and providers and does not promote or recommend any provider. Consequently, HHSG cannot guarantee that a transaction will occur or if it does occur that it will satisfy the needs of either user or provider. All fees associated with the Referral Service are paid by the network providers. Specific Prohibited Uses the Site may be used only for lawful purposes by individuals seeking elder care, senior care, home care products and services, employment and employers seeking employees. HHSG specifically prohibits any use of the Site, and all users agree not to use the Site, for any of the following: Posting any incomplete, false or inaccurate information or information which is not your own.
