Personal Care Services

Home Health Solutions Group staff provides in-home services to our clients at the level they require. Based on the needs of the client, our Clinical Director visits the client and develops a Care Plan, then our office personnel implement the services required with our experienced and qualified staff.

Personal care may include help with the daily activities of the household as well. We provide Grooming and Hygiene, Mobility Assistance, Walking, Transferring and Repositioning, Toileting and Incontinence Care, Dressing, Brushing Hair, Shaving, Nutrition, Medication reminders, Shopping and Meal Preparation to make it easier for clients to maintain good nutrition while living independently in their own homes. Laundry and House Cleaning can also be included in our Personal Care plan. Our goal is to help clients maintain the quality of life they’re used to, despite their declining abilities.

Because needs can change, we revise and update the care plan and incorporate different services as needed. Our staff includes Home Health Aides, Certified Nursing Assistants, Licensed Practical Nurses, Registered Nurses, Specialty Care Nurses (Hi-Tech Nursing, IV Therapy, Wound Care), Physical, Occupational and Speech Therapists, so that we can provide a wide range of in-home care services.

Grooming and Hygiene

Home Health Solutions Group’ goal is to assist our clients with their personal care routine so they can look and feel their best. Studies have proven how effective and positive it is for elder, older adults and seniors to have regular hygiene and grooming. For clients who fear falling while bathing or have physical or cognitive limitations that make bath-time difficult, our caregivers are there to help. We can assist with shampooing, shaving, hair combing and styling, nail and skin care, oral care and other grooming needs

Mobility Assistance

Remaining active in your golden years is one of the best ways to maintain the quality of life. Our caregivers help elders, older adults and seniors remain active by assisting them in getting from place to place in comfort and safety.


Transfering and Repositioning

One of our top priorities is helping our clients move around their homes. This may include getting in and out of bed, chairs, and wheelchairs. Proper transferring and positioning can contribute to reducing bedsore risks. This also helps reduce muscle atrophy, while encouraging proper breathing and digestion. It can also lower the risk of falls and injuries, as well as keep our clients as comfortable as possible.


Toileting and Incontinence Care

We understand the sensitive nature of toileting and incontinence care for our clients. Without proper help, elder, older adults and seniors can face many issues. These may include skin infections, unpleasant odors, and general hygiene problems. Our caregivers offer compassionate, dignified help with these personal aspects of daily living.

Nutrition

Maintaining a healthy, nutritious diet can affect well-being and vitality across the board. In fact, many of our elders, older adults and seniors have prescribed meal plans they must stick to for health reasons. Whether you are looking for help adhering to a special diet or just want to improve nutrition, our caregivers can help. They also provide companionship, turning meals into social time by engaging with elder, older adults and seniors while they eat. The positive environment helps them feel more relaxed. Supervision also makes certain your loved ones get proper nourishment. Our caregiver’s help certain clients stay on prescribed diets to prevent or control a wide variety of medical conditions like heart disease, diabetes, and osteoporosis. Our caregivers also help clients eat nutritious menus to promote disease prevention, healing, and healthy living. We make an extra effort to turn meals into enjoyable social time by engaging our clients in conversation over food.

Medication Reminders

Although our caregivers cannot administer medications, they can assist in ensuring that clients take their medication as prescribed. Our caregivers can help with opening medication containers, reading labels, and reminding the client when it is time for the next dose. Because our caregivers spend considerable time with clients, they are often the first to notice changes in physical and mental health. Caregivers monitor client status by direct observation, and by noting and recording vital signs under the direction of health care professionals. Observation and Reporting – This is one of the most important services caregivers do. It includes monitoring a client’s status, and reporting concerns and changes to a supervisor regarding the client's physical well-being, functional level, cognitive status, skin condition, mood, mobility, interactions with others, and eating and sleeping patterns. Vital Signs – Most Family Resource caregivers are trained to monitor and document vital signs, including pulse, temperature and respiration




Shopping

Some clients might want to be able to go to shopping or choose their own things, that way the feel and are more independent. Our caregivers can help taking clients to shopping.

Meal Preparation

Daily tasks, like meal preparation, can be a lot of work for elders, older adults and seniors. Our caregivers’ step in and help in the kitchen when needed. They cook hot and wholesome meals to meet nutritional and dietary needs. Your loved ones are always welcome to work alongside their caregiver during meal prep. This time spent working together is an excellent opportunity for social interaction and conversation.

Laundry and House Cleaning



Our caregivers can take care of all laundry needs, either in the home or at a nearby laundromat. These services may include washing, drying, ironing, and putting clean laundry away. Home Health Solutions Group caregivers are available to provide light housekeeping. Our services include vacuuming, dusting, sweeping, mopping, cleaning bathrooms (sinks, tubs, showers, and toilets), cleaning kitchens (sinks, appliances, counters, and taking out the trash), straightening all rooms, organizing closets and drawers, and cleaning any interior windows reachable without a ladder.
