Personnel Requirements

  • Home Health Aide Certificate (75hrs) or CNA certificate
  • BackgroundScreen Level 2 (every 3 years)
  • OSHA certificate (annual)
  • HIV/AIDS Certificate (biannual)
  • Alzheimer Certificate (biannual)
  • Domestic Violence (biannual)
  • HIPPA (biannual)
  • Patient Rights (biannual)
  • Current physical within previous 6 months stating “free of communicable disease" (annual).
  • Current PPD (required annually).
  • Current CPR
  • Social Security Card
  • Proof of Residency or Citizenship (Work permit/Alien card)(Voter registration or Passport)
  • Driver License or Florida ID
  • Auto Insurance
  • Professional Liability Insurance
  • 2 Reference letters and/or verification of employment contact



ALL DOCUMENTS MUST BE CURRENT PRIOR TO REGISTRATION.
